How do I use my benefits?

Delta Dental PPO Plan:

  • Member ID: Your member ID is the student ID you provided when you enrolled.  If your Student ID is less than nine numbers, please add zeros to the beginning to get it to nine numbers.
  • A dental ID card will be mailed to the address you provided during enrollment.  However, you do not need this ID card to see a provider.
  • The provider will be able to find you in their system by your name, date of birth, and 9 digit student ID.
  • Once enrollment is activated, you may register for the Member Connection, where you can print a temporary ID card, view claims information, and enroll in the Enhanced Benefit Program. Click here for instructions on how to register for the member connection Registration with Member Connection is not necessary to utilize your benefits. 
  • Delta Dental also has a mobile app. Click here for details.


DeltaCare DMHO Plan:

  • Under the HMO plan, a facility ID needs to be entered at the time of enrollment.  
  • To find a facility go to Under “Select your Network” chose DeltaCare.
  • Rosters are generated at the beginning of each month. You will need to register by the 15th of the month to appear on the next month’s roster.


DeltaVision Plan:

  • Your DeltaVision plan uses the EyeMed Select network. A vision ID card is not required to access service. Your vision provider will need your name and date of birth to verify coverage.
  • To find a provider go to  Enter your zip code and under “Chose Network”, pick “Select”.
  • You can print this DeltaVision PDF to be sure you have your plan information handy when you visit your provider.
