How to find a participating provider:

  1. Click here for the Delta Dental provider search.
  2. Delta Dental Premier PPO Plan – Under “Select your Network” choose either the Delta Dental PPO or Delta Dental Premier network.

Pro Tip: The Premier PPO Plan allows you to use any dentist you wish, but you will receive the greatest benefit by using a dentist who participates in the PPO network.

DeltaCare USA DHMO

How to find a participating provider:

  1. Click here for the DeltaCare USA provider search.
  2. Enter an Illinois Zip code. Under “Select your Network” choose DeltaCare USA. At the time of enrollment, you need to enter the six digit Facility Code associated with the Illinois provider you selected. Should you need to update your DeltaCare USA facility ID, please login to your DeltaCare USA member portal or call Delta Dental Customer Service at 800-639-0327.

Pro Tip: The facility must be located in Illinois. Should you need to see a specialist, your primary dentist will provide a referral.

How to find a participating DeltaVision provider:

  1. Click here for the EyeMed Select network search.
  2. Enter your city or zip code, and choose your network to generate a list of vision care providers near you.
